St. Monica School Registration Information - 2017-2018
THE FOLLOWING THE REGISTRATION DATES FOR NEW KINDERGARTEN THROUGH
EIGHTH GRADE STUDENTS:
***Students entering Kindergarten must be 5-years-old by September 1, 2017
Thursday, February 16th 9:30 AM - 11:30 AM in the Senior School Office (2500 S. 16th Street)
Thursday, February 23rd 6:30 PM - 7:30 PM in the Junior School Office (Entrance next to Church)

REQUIRED DOCUMENTATION FOR ALL NEW STUDENTS
* Birth Certificate * Baptismal Certificate if Catholic (unless Baptized at St. Monica's) * Up-to-Date Immunization Record - including varicella (chicken pox) * Social Security Number * Copies of most recent report card and standardized testing (Grades 1-8) * Registration Fee of $100 per child up to three children * * An entrance test will be administered to all students entering Kindergarten. This test will be administered in late May/early June. Parents will be notified of the test date and time.
REGISTRATION WILL NOT BE PROCESSED WITHOUT THE ABOVE DOCUMENTATION.
NO STUDENT WILL BE ADMITTED IN SEPTEMBER WITHOUT UP-TO-DATE IMMUNIZATION RECORDS.
